Use this form to add, change, or cancel a direct deposit. Please complete the Direct Deposit Authorization form and send it by mail to Housing Choice Voucher Department, 315 N Columbus Street, Lancaster, OH 43130 or fax to 740-653-7600. All changes must be in writing to the Housing Authority no later than the 21st of the month before the desired month in which the direct deposit is to be processed, i.e. if we receive your direct deposit authorization by May 21, your direct deposit will then be effective June 1.
W-9 forms must be updated annually throughout landlord's participation with the Housing Choice Voucher Program.
